Copies of Czech Records

The Embassy of the Czech Republic in Washington, D.C. no longer processes genealogy research requests. For information on how to proceed with genealogy requests, refer to Using the Archives.

However, if you were born in Czech Republic and wish to request a copy of your own birth certificate, you may contact the Consular Office of the Czech Embassy for details, or see the Embassy's Contact Information.

Application for Czech Birth Certificate

If you require the first Czech birth certificate of a child born abroad, you can follow the instructions for Application for the issuance of a Czech birth certificate (in Czech language).

Application for Czech Marriage Certificate

If you were married while abroad, it is necessary to have the marriage registered in Czech Republic as well, by following the instructions for Application for the issuance of a Czech marriage certificate (in Czech language).

Copies of Slovak Records and Slovak Genealogy Request Forms

The website of the Embassy of the Slovak Republic in Washington, D.C. includes a page about Genealogical research sources and requests. Genealogical research can be requested regarding documentation older than 30 years either in the form of a running account, or in the form of an individual vital statistic record. The relevant application forms via a running account application, or via an application for individual vital statistic record (birth, marriage, death) are available for download on the embassy's Genealogical research website page.  Both forms are bilingual (Slovak & English).
